Appryse Website (Online Athletic Forms)
Please read this entire page before enrolling online
1. Appryse Background
We are very excited to continue Junipero Serra's second school year using Appryse. Appryse is a web-based solution which is a replacement to the paper-based system previously used to fill out medical history, contact information, and release forms.
As parents, you will now be able to enter all information online and upload or fax doctor physical forms. Since a new physical and updated medical information is required for every school year, the online website of Appryse will simplify the process.
Information can be updated throughout the year as it changes, and next season you will only have to review and update the information. Improving privacy, coaches will now use a smartphone instead of carrying binders.
In the event of injury, you will be automatically notified via phone, email, and text message, simultaneously.
All information such as medical history, doctor's physical, and parental consent forms will be handled within the Appryse website electronically.
2. Important notes before you enroll
***IMPORTANT NOTES #1:
Your son must be enrolled on Appryse and cleared before being allowed to tryout for any sport. Do not wait until the last minute, as this will cause your son to miss tryout days.
If you are having any problems registering on the site, please contact Appryse support by clicking the "Contact Support" button on the Appryse website. DO NOT CONTACT Junipero Serra High School.
***IMPORTANT NOTES #2-
Freshmen DO NOT need to provide physicals on this site as they turn in physicals as part of their initial enrollment at Serra. Please ignore the pending doctor's physical requirement if your son is a freshman.
***IMPORTANT NOTES #3-
Do NOT fax in your own doctor's form as this will not be recognized by the Appryse system and will NOT be received.
It must be the approved Appryse doctor's physical form and must include the Appryse bar code included on the Appryse doctor's physical form!
DO NOT BRING THE APPRYSE PHYSICAL FORM TO SERRA HIGH SCHOOL, as this is the whole point of online documents!
Please either have the parents or your doctor fax the Appryse physical form to the number at the top of your Appryse physical form, or upload it directly to the Appryse website.
To enroll your son FOR THE FIRST TIME as a freshman or to RE-ENROLL your son as a sophomore, junior, or senior on the Appryse website please click this link: https://app.appryse.com/enroll/AYUCSC2J
You must enroll on a PC or MAC, not on a smartphone!
The only paper form that is mandatory to turn into your coach is the Student Athlete permission form which gives Junipero Serra High School your permission as a parent to use your electronic signature.

Please make sure your son hands this form to his coach, do not turn it into the front office.
4. Forms Requirements
The following is required once every school year:
1) A new Appryse only physical form signed by a doctor every calendar year for sophomores, juniors, and seniors clearing your son for athletic activities FAXED to the number provided at the top of your Appryse physical form, or UPLOADED to the Appryse website.

2) Updated medical history and review of concussion/steroid policies on the Appryse website.
3) One Parental consent for all sports in which your son is participating which will be sent to you via email once your son's enrollment is approved. You must give your parental consent for your son to participate in all sports at Serra High School.
4) Football Insurance Forms 2015-2016-Coming soon!
Please make the check out to Myers, Stevens, and Toohey and mail the check directly to them, do not bring the check to Serra High School!
The football insurance consent form must be turned in to the coach, even if turning down the additonal coverage.
