Course Registration Form

• The Freshman course registration form is located in your son's application portal. You can log in HERE. 

• Once logged into your portal, Use the top navigation bar to locate "my course registration". Select My Course registration and complete video and form.

• Course registration form due friday, April 3, 2026. class selection is prioritized based on completion date of course registration form.

Incoming Freshman Physical Form Requirement
 

all incoming freshmen are required to complete and submit a physical form, which can be found on the athletic clearance website. Accounts for submission may be created starting May 1, 2026. Please ensure you select the "2026-2027 school year" and the correct school: Junipero Serra/san mateo. 

• for students participating in summer sports:  the Physical form must be sumbitted to the athletic clearance website before the start of the summer program.
• for students not participating in summer sports, but planning to play a sport during the school year: the physical form must be submitted to the athletic clearance website no later than Tuesday, June 30, 2026.
• if you do not anticipate playing a sport freshman year, please email the form to  The admissions Office.

• Immunization Records: You are required to provide us with a copy of your son’s most current official immunization record by Tuesday, June 30, 2026. It must include proof of the Tdap booster shot required of all California students.

• Immunization records should be emailed to the admissions office.

All Serra Padres are expected to complete reading and math assignments over the summer break. Please click on Summer Reading and Summer Math to view assignments due in the fall. Summer assignments will be posted on June 1, 2026. Please wait for your math placement before beginning any summer math assignments.
