Physical Form

• Every incoming Freshman/transfer is required to complete the physical form.

• All physical forms must be submitted by Friday, July 23, 2021.

• If you plan to play a sport during the school year, the form must be uploaded to the athletic clearance website. 

• If you do not anticipate playing a sport, please email the form to admissions@serrahs.com.

• Immunization Records: You are required to provide us with a copy of your son’s most current official immunization record by Friday, July 23, 2021. It must include proof of the Tdap booster shot required of all California students.

• Immunization records should be uploaded into Ravenna using the Immunization step located on your preliminary application page.

Course Registration

• Course registration for transfer students is done on an individual basis based on academic record. A few days prior to your May 13 Welcome and Course Registration event, you will receive your course outline for the 2021-2022 school year. You will be able to ask questions or request changes during the event.
