Athletics Clearance Process

Serra uses a web-based provider to collect and maintain a student’s medical history, contact information, and release forms. Below is a link to download the Pre-Participation Physical Form, which includes a medical history to be completed by parents and a physician’s evaluation to be completed, signed, and stamped by a licensed medical physician.
  • A physical exam is needed for any Serra student who plans to participate on a Serra sports team anytime during the 2023-2024 school year
     
  • Go to: Athletic Clearance to create an account and to complete the process. Please make sure you select the correct school (Junipero Serra/San Mateo) and select school year 2023-24 when you begin the process.
     
  • The physical must be signed and stamped by a licensed medical physician and can be uploaded to your online account or can be emailed to Athletic Director Justin Ferdinand. Hard copies of the signed physical may also be delivered by hand to the Athletic Office. Main Office Staff will NOT accept hard copies of physicals.
     
  • This process must be completed and you must be cleared by the Serra Athletic Department prior to trying out or participating in any sport.

How to use Athletic Clearance


Important Note: The California Interscholastic Federation (CIF), the governing body for high school sports in California, requires that student athletes have a current annual physical examination, or statement by a medical practitioner, certifying they are physically fit to participate in athletics before they are eligible to try out, practice, or participate in interscholastic athletic competition. The date of the certification must be within 365 days of the completion of the sports season.
